Single Individual Player Applications and Other Forms
For All Individual Players aka "Free Agents" that are looking to sign up with a team.
We accept all Free Agent players as first come basis. All Free Agents that are signed up are placed on the Active Player List in order of completed applications received for teams to choose players from. -The Individual Player Fees are located on our PayNow Season Fee page. -If you are picked up by a team, then the coach will contact you. -If you are not picked up by a team, then you may stay on the list or go on a waiting list for next season. If you choose to remain on the Active Player List then if a team needs a permanent player, they will pick from that Active Waiting List first. We cannot guarantee placement if you sign up but we make every effort to place our players.

" Free Agent" Spring/SummerApplication is now OPEN This Active Player Packet located below these instructions are for all new individual players or all returning individual players that would like to sign up to play. ~Please print off each form and fill out completely if it applies. ~See Office Hours and Location to bring your completed Packet by or mail to the Office. ~If you are looking to sign up with a particular team, you must contact that coach prior to being added onto that team. ~U.S. AMATEUR INSURANCE FORM REQUIREMENTS- Any players that DID NOT play Fall, or Winter I, II or III Indoor season must fill out the US Amateur Insurance Form and turn it in with application. (Player ID is your Drivers License Number.)
~U.S. SOCCER FEDERATION INTERNATIONAL CLEARANCE FORM REQUIREMENTS- Any players that would like to play but are not US Citizens are required to fill out the International Clearance form and turn in with thier US Amateur Form.
PLAYER CARD - Please bring a COLOR copy of your Driver's License with your Packet to make your Player Card or we can make a copy of your DL here at the office. If you already have a player card, please bring it to the office for updating or to make a new one.
