Team Application/Roster Instructions
SPRING/SUMMER SEASON TEAM ENTRY FOR Spring/Summer IS NOW OPEN. TEAM APPLICATION DEADLINE IS Feb 28th. ALL PLAYERS MUST PAY INDIVIDUALLY TO BE ON ROSTER. See Application for Season Fees and for payment at PayNow Season fee web page Sign Up during Office Hours only. See Office page for hours.
~Located at below is the Team Application,Team Roster, US Amateur Insurnace Form** and International Clearance Form*. ~ We will not accept incomplete applications!! FOR NEW TEAMS- TEAM APP/ROSTER DEADLINE- Feb 28th -To be considered complete, Team App must be filled out and signed by coach, Team Roster must be typed out and signed by all players. All information on roster must be typed except for signature area. The roster is in Excel format and Player information areas are unlocked for you to type in and then print off once you have filled in all required information. Player Signatures are to be handwritten by player. Players will only be added to your roster when their season fee is received by office, any USASA fees, any players USASA forms, any required International forms and Cash/Cashier's ck for Bond Fee must all be submitted to office be eligible for sign up. Please see Requirements for USASA Amateur Forms/Fees and the International Form below. RETURNING TEAMS- TEAM APP DEADLINE- Feb 28th- No new roster is required just Team app. Your yearly roster is on file at office. New players to your team must sign the team roster on file at office and turn in any USASA forms/fees or International forms before playing. ~Contact one of the Directions for more information or questions on Team Sign Up.
**United States Amateur Soccer Insurance Form Requirements for this form- It is to be filled out by each new player for the Season and each player must pay the $25.00 USASA insurance fee. They are to be included with your Team App. * United States Soccer Federation International Clearance Form Requirements for this form- For any player that wants to play but is not a US Citizen. Please submit with the US Amateur forms and Team App.
Player Card must be brought with you to each game! It is each player's individual responsiblity to bring their card to each game. You CANNOT for any reason play without your card. You must have the Current Year Sticker attached to it to play. Player Cards can be made or renewed through the Office. This card shows that you are a paid, approved and Insured member of the United States Amateur Soccer Association. It is given to the Referee at the beginning of the game and it will be returned to you after the game. If you receive a red card or have outstanding fees, your card will be held until fine/fees has been paid. Remake fee will apply if you have lost or damaged your card.
